Creating Forms in Microsoft Access

In this lecture you will learn how to create forms in Microsoft Access. Forms are used to input, view, and modify data in a nice graphical way. Using forms you will be able to build a simple applications in Access that can be used by people that do not know databases.

The presentation (full size , small size) is based on Microsoft Access 2007. If you are using Access 2003 or an older version, some options may look differently, or be accessible in other places in Access.

Please listen to the recorded presentation. Note that only basic Access forms options are used in the presented example. One may want to create advanced graphical interfaces suitable for advanced databases. Additionally, forms can be combined with macros programmed using Visual Basic (this is out of scope of this lecture).

Build Your Own Forms

Practice what you learned in this lecture by creating your own forms for entering and viewing patients, physicians, visits, and diagnoses information in the example database. You can use already prepared database in Access 2007 format, or create your own database from scratch. Create forms for:

  1. Entering patient information, including addresses
  2. Entering physician information
  3. Entering visit information
  4. Entering diagnosis information

Use your forms to insert at least two records to each table. Email your instructor with screenshots of your forms and data.
